Stefanie Lord, M.D.
Executive Medical Director
Dr. Lord is a board-certified internist who joined the Haven Medical Group in September 2014. She cared for patients at North Florida Regional Medical Center for 17 years and last held the position of director of the Chronic Kidney Disease Clinic for Nephrology Associates of North Central Florida.
Also during her time with North Florida Regional Medical Center, she served on the Quality Assurance Committee and as chair of the Pharmacy and Therapeutics Committee. Her professional activities at North Florida Regional Medical Center also include chief of staff (1999-2000), vice chief of staff (1998-1999) and chief of the Department of Medicine (1996-1997).
Dr. Lord earned her medical degree at the University of Florida in 1988 as well as a bachelor of science in microbiology.
Ellen Laura Talos, M.D., HMDC
Executive Medical Director
Prior to joining the Haven Medical Group in June 2011, Dr. Talos had her own private practice in family medicine at the Putnam Community Medical Center. She holds a certification as a hospice medical director and is currently an affiliated clinical assistant professor at the University of Florida College of Medicine.
In 2009, Dr. Talos was named 2009 Resident Teacher of the Year at Beaumont Hospital in Grosse Pointe, Michigan, and in 2008, she was the recipient of the Beaumont Hospital SEMMES/Resident of the Year Award.
Dr. Talos has served as a volunteer physician at the Gary Berstein Community Health Center and was a volunteer research assistant at Grady Hospital Emory School of Medicine. She is a member of the American Academy of Hospice and Palliative Care and the Putnam Community Medical Society. She earned her medical degree at St. Christopher’s College of Medicine in Dakar, Senegal (Luton-England Campus) in 2006.
Neel G. Karnani, M.D., FAAFP, FAAHPM
Associate Medical Director
Board certified in hospice and palliative care as well as in family medicine, Dr. Karnani joined the Haven Medical Group in 2006. He is a fellow of the American Academy of Family Medicine, a delegate to the Florida Medical Association, a fellow of the American Academy of Hospice and Palliative Medicine, and board chair for the Indo-American Medical Association of Jacksonville.
From 2002 to 2006, Dr. Karnani joined the University of Florida College of Medicine faculty as the division chief in the Department of Community Health and Family Medicine at UF Health Jacksonville. Having received his medical degree in 1978 from the Armed Forces Medical College in Pune, India, Dr. Karnani pursued post-graduate education in otolaryngology in England for three years. Subsequently, he completed a residency in family medicine in Jacksonville, Florida.
In 1996, after several years of private practice, Dr. Karnani joined the faculty of the Family Practice Residency based at St. Vincent’s Medical Center in Jacksonville. Dr. Karnani has been published in a variety of medical journals and magazines since 2003.
More about Dr. Karnani: Dr. Karnani speaks English and Hindi with some conversational ability in French and Arabic. His interests include playing tennis, golf and running as well as listening to music, playing bridge, reading and enjoying ethnic foods.
Geraldine S. Bichier, MD
Associate Medical Director
Prior to joining the Haven Medical Group in February 2008, Dr. Bichier held the position of assistant medical director at the Center for Clinical Trials Research at the University of Florida where she was intimately involved in the screening and study of volunteers while ensuring the safety of subjects and interpreting clinical data.
She earned her medical degree from the University of Wales in the United Kingdom. Dr. Bichier was a fellow in nephrology at the University of Florida, completed her residency in internal medicine at UCSD Medical Center in San Diego, California, and she served patients in a nephrology and internal medicine practice at the Valley Nephrology Medical Group in Fresno, California.
Previously, she was a registrar in cardiology, nephrology, neurology, gastroenterology and pulmonology for several hospitals in the United Kingdom.
More about Dr. Bichier: Some of Dr. Bichier’s interests include gardening and horseback riding. During her free time she also enjoys exploring Payne’s Prarie in the Gainesville area.
Patrick F. Woloszyn, M.D.
Associate Medical Director
Prior to joining the Haven Medical Group in June 2014, Dr. Woloszyn provided outpatient, family practice care and medical oversight for public health concerns as the Suwannee County Health Department Medical Director for 20 years.
Prior to his position at the Suwannee County Health Department, he was a medical consultant for Quality Care Assurance from 1994 to 1995 where he provided consultation to long-term care facilities, conducted risk management reviews and assisted in legal case preparations. For eight years, he served as a long-term care physician at the Texas Department of Health.
In 1978, Dr. Woloszyn earned his bachelor’s degree in chemical engineering at Clarkson University in Potsdam, New York, and earned his medical degree from the University of Texas Medical Branch in 1985.
Duaine Murphree, M.D.
Dr. Murphree is board certified in family practice and was a faculty physician with the St. Vincent’s Medical Center Family Practice residency program prior to joining Haven Medical Group. He has practiced privately with the Jacksonville Health Care Group and has been an independent contractor working as a hospitalist with Baptist Primary Care.
He earned a master’s degree in clinical epidemiology at Wake Forest University Bowman Gray School of Medicine and his medical degree at the University of South Alabama.
Danielle DiGennaro, MSN, ANP-BC, Lead ARNP
Immediately prior to joining to Haven, Danielle focused on home palliative care as a nurse practitioner for the Palo Alto Medical Foundation in Fremont, CA. At the beginning of her career, Danielle spent 7 years (5 as an RN and 2 as an NP) practicing inpatient cardiology at Duke University Medical Center. Her focus was advanced heart failure.
More about Danielle: Danielle enjoys biking with her toddler and visiting child-friendly events in north central Florida. Danielle also enjoys knitting in her spare time.
Lily Allin, MSN, FNP
Prior to joining Haven, Lily was on staff at the Hematology Oncology BMT Unit as an ARNP at University of Florida Health Shands Hospital in Gainesville while she continued working as a middle school teacher at Alachua Learning Academy. Prior to that she worked as a registered nurse for Johns Hopkins Hospital in Baltimore, MD in the Oncology/Hematology unit and in the outpatient clinic. She has also worked at NFRMC’s ICU.
More about Lily: Some of Lily’s interests include going for walks and doing yoga. She also enjoys listening to books online and cooking.
Cynthia L. Boucher, MSN, ANP-BC
Before joining Haven, Cindy was the clinical coordinator and ARNP for the Hepatitis C treatment program at the University of Florida Hepatology Clinic. She also served as an associate professor of nursing in the Santa Fe College ASN program for nearly 10 years. During her tenure, her focus was on medical, surgical and acute care. She taught clinical for ASN students on a cardiac step-down unit. She continues to precept ARNP students from the University of Florida and University of Southern Alabama.
More about Cindy: Cindy enjoys spending time with her two children on trips to Walt Disney World, the beach and the mountains. Her hobbies include crocheting, gardening and cooking.

Kathleen A. Russell, MSN, ANP-BC, ACHPN
Prior to joining Haven, Kathleen worked as a staff registered nurse at Community Hospice of Northeast Florida, Florida Hospital HospiceCare of Ormond and St. Vincent’s Medical Center. She earned her master’s degree in nursing from the University of Alabama.
More about Kathleen: Kathleen likes to enjoy the beauty of nature with her granddaughters Lilyanne and Anna. She also enjoys dancing, swimming, cooking, baking, gardening and reading.
Mary K. Schupp, DNP, ARNP, FNP-BC, CUNP, ACHPN
Mary is a family nurse practitioner and doctor of nursing practice, certified in hospice and palliative care, with over 35 years of clinical experience in most fields of nursing including management, education, home health, critical care, post anesthesia care, and medical and surgical care, including twelve years of advanced practice evaluation and management skills. Prior to joining Haven in 2015, Mary practiced as a certified urology nurse practitioner for Urology Associates of Northeast Florida, and Advanced Urology Institute, while promoting an intravesical treatment option for the management of chronic cystitis.
More about Mary: Mary enjoys spending time with her husband rowing along the Bayfront in St. Augustine taking in the sites from the Intracostal Waterway. She also enjoys giving her grandchildren lessons in African drum, piano, weaving, Kung Fu forms and dance.
El Agathe Tembeng, MSN, ARNP-BC
Previously, El Agathe was an ARNP at My Home Doctor and a rounding nurse practitioner at Fish Memorial and Central Florida Regional Hospital in Sanford, Florida. An adjunct nursing instructor at Bamenda University of Science and Technology and fluent in the French language, she earned her master’s degree at South University in Savannah, Georgia.
Laura Yantes, RN, MSN, FNP
Prior to joining Haven, Laura was a family nurse practitioner at Advanced HealthCare and a registered nurse/assistant manager for Progressive Care–Baptist Medical Center South. She was the 2012 recipient of the Baptist Medical Center South Excel Award and the 2011 recipient of the Baptist Medical Center South Transformational Nursing Award.
More about Laura: Laura enjoys working out and anything to do with the water, specifically boating on the Intracostal Waterway to St. Augustine.
Katie Youngblood, DNP, ARNP, FNP-C
Rita Long Arrington, MSN, ARNP-BC, FNP
Rita previously worked as a nurse practitioner for Haven Hospice. She also spent time as a family nurse practitioner in a primary care practice in Williston, the Chiefland Medical Center, Dixie County Health Unit and Gilchrist County Health Unit delivering care to patients of all ages. She has served as a preceptor for nurse practitioner students.
